I need help setting up spreadsheet

who can help me create form inside a port that the employee can modified. Like a members portal. The information they've completed on an initial form would populate in a memeber only portal. 

The form is possible with JotForm. However, we will need a few more information regarding the members-only portal that you've mentioned. Is that an external service provider? We do have CRM integration in the form builder if that's what you mean by the portal.
