How can I make the multiple choices be for one person per form?

Hello, on the form I have a list of times for customers to choose, once chosen I want those options to be gone for the next customer filing out the form.

So you mean to say that in a single form you want multiple persons to be able to fill out. Do you intend to have multiple pages where you can take in data for one person per page?
