HIPPA Application

    I am trying to view a submitted test form and this notification pops upAll healthcare organizations, nonprofits, and government organizations must fill out the application form to apply for the Coronavirus Responder Program. The program is free for new JotForm users only — if you’re an existing paid user and get approval for the Coronavirus Responder Program, you will continue to pay your usual fee, but your account will become unlimited at no additional charge. This offer is not available for Enterprise users. The Coronavirus Responder Program will end August 1, 2020. It will not close and allow me to apply and I cannot see my form. I can however continue to work on the form in progress that I am testing. Please advise

    I think you must have been logged out from your account. Please try to log in again to your account then click this link to apply: Covid19 form. Once you are logged in the username is prefilled then fill out the other information on the form.

    If the issue still persists kindly try to use a different browser or clear your browser's cache. Let us know how it goes.
