My account has been suspended

    My account has been suspended due to allegedly phishing.

    But I upon checking, my form was legit.

    Pls help me re activate my account as I have clients, filling up those forms.

    Thank you for your message, after checking your account, I can see that has been suspended due to the form: https://www.jotform.com/form/200898117242051, since is collecting sensitive data under the field: Upload of Valid ID.

    If you need to collect personal information, please follow the instructions below:


    1) Insert a Terms & Conditions widget into your form with your Terms of use

    2) Insert a Signature widget into your form so your respondents can acknowledge that they are providing sensitive information

    3) Change the email address on your profile to a corporate one.

    4) Send an email to reviewers@jotform.com with proof of your affiliation to a legitimate business and request a review of your account.

    I already reactivate your account.
