You can add two Spreadsheet to Form widget that would be shown according to the language selected.

For example, you can have these two conditions in the form:

Each widget will have respective language data into it. For example, If your form fields are translated into French, you also have to translate the column names in the Spreadsheet file, to match the translated labels. So, you need to use files, one for the English and one for the French version.
