How can I insert a code verification tab in the form?

I'm creating a form which needs to include a code verification system.

Some details below:

- Members will receive a unique code and link to the jot form. In the form, they'll input the unique code, their name, and their email.

- The code will be unique to the member, and can only be used once. 

Questions:

- Can Jotform record when a code has been used which matches a database of codes?

- Can Jotform prevent further use of the same code?

I would suggest you to use the SMS Confirmation widget. You can find it here:

This widget will allow you to send to your users a confirmation code via SMS message. It is powered by Twilio You just need to enter your Twilio account credentials into the widget settings to get started. When a user provides their phone number, they will receive a verification code, which they can use to continue filling out your form.
