My account is locked?

    Hello, I am using this form to create a basic technical skills self-assessment. I am unable to get back into my account to access the form and continue editing. It says my account has been locked. 

    Please assist - thank you!

    Hello,

    When I try to log in, I get an error that my account has been suspended by the phishing prevention program. I am creating a technology skills self-assessment with a drop-down menu for each question. The form does not ask for any passwords. It asks if users are comfortable changing their passwords, logging back in to various programs after a password change, etc.

    Please help - I need to get this completed ASAP.

    Hello, and thank you for the reply. I have been trying to access this form to get it completed since 9am and it is now 12 noon. I need to get this out to staff today if at all possible. Please, if you look at the form, it is very clear that all questions are basic work tasks, and the only response is a drop-down menu for their comfort level with each task: 1-Very Uncomfortable to 5-Very Comfortable. There is no request on the form for *any* response outside of the drop-down menu.
