Why was my account suspended?

    I logged in to find that my account was suspended. First off, why did I not receive an email about this?

    I have only a form for job applicants on my website. People apply on Indeed or Craigslist and I direct them to my website to complete the application form. How could this possibly be labeled as phishing? It only asks basic information.

    Please help me resolve this immediately as it is preventing me from continuing to grow my business.

    Our Phishing Detector automatically suspended your account because of the following form: "Request for Taxpayer Identification Number and Certification".

    You collect SSNs in this form. Please note that this is not allowed and it violates our TOS. You can read it here:

    If you do not collect this data from the general public, but from your own clients, you can collect SSNs. You need to comply with two things:

    1. You should add the Terms & Conditions widget (or the Short Scrollable Terms widget) for your clients to read and agree.

    2. You should add a Signature widget. So, your clients can sign and agree to provide such sensitive information.

    Please have in mind that it is only allowed to ask for social security numbers. You can not ask for any kind of file uploads for this document.

    If you want to reactivate your account, you should agree to add the above widgets. Or we can just delete the offending field.

    Please tell us how to proceed.

    I believe I pulled that template from the Jotform forums... 

    Please remove it from the account. It is not actively used.

    I really need this to be resolved ASAP. I am unable to hire and I have potential applicants asking how to apply.
