Please see attached screen shot. I have a lot of wasted space and I want to make the questions wider on my submission. I do not find a place to change this.

I also want to know the name of the Submission Date field so I can use it in a text field and put a label on it.

Lastly, Is there anyway i can apply different colors to my answers based on the answer like you all did on your default submission PDF?

I am not able to check PDF document of your form because of the HIPAA compliance. I am not sure what is causing this alignment issue in your PDF document. Please change the PDF alignment (not field text alignment) and see if that solves your problem:

Pease note that there is no separate tag for submission time. If you want to show submission time in form, I would suggest you to add "Date Picker" field in form with time enabled and set it to display current time as default time. You can also hide it form form as well. You can then add this field in desired location of PDF document. Please check the screenshot below:

Unfortunately it is not possible to conditionally change the background color of answers.

    Hope this helps.

i did not ask about the submisson time. Did you read my question. I was trying to find out the name of the submission date field so I can add it within a paragraph or text field and put a label before it that states "Submitted on: {field name}"

Since you were unhelpful i guess i have no choice but to add a field to capture the date...

I still want to make the label/question column wider. you ignored this in your response.....

You can only add the submission date using the option on the "Add Element" section:

Thus, the variable {submissionDate} or any other to get this data does not exist, the workaround about adding a date picker with the default date to the form is valid so you can use the field name on your text elements as desired.
