People can't type in the fields for quantity or lead time anymore. How do we fix this? Thanks.

Hi, thank you for reaching out to us
As per checked on your form, I noticed that you have set a condition to automatically calculate the field value once it is filled. This will make all the users' input overridden by the condition. I suggest that you will disable all the conditions to make the fields working again.
Let us know if you have any other questions. -

Hello there,
If you are referring to using an input table for collecting numerous data then yes, compared to using individual short text fields which would tedious to build.
If you meant putting the form on the landing page, then I would not recommend it since it would make the entire page long, I would suggest that you create a link to the form page for submissions instead.
I hope that helps.
Best.
