I set up an account. spent hours on a medical history form and covid19 screening.
I start to use it and account is disabled for "phishing"
Please fix this issue.

no one has answered within the last 30 minutes.
I have given this form to a dozen clients.
This is really poor.
Please can you reactivate the "Dentistontherock" account.
If there are amendments tell me but I cant even access
