Spam: is there anyway you can block the account so my information can possibly be deleted?

Hello, this morning I received a JotForm to fill out for a house rental and have not received any information back from the person who sent it. I also called and my number got blocked. I believe this may be spam. Is there anyway you can block the account so my information can possibly be deleted?

We've taken down the form and suspended related account. Person that was collecting the data using that form won't be able to access information you submitted.
