Issue with filling in check boxes

Need help with form. I have created a questionnaire with one of your forms and there is a section that has fillable blocks. I would like for people to be able to put an x in the block but it will only allow me to put in numbers. What will I need to do?

Is there a better form that I can use for the type of form I am trying to create? Thank you so much.

To fix the issue, change the FIELDS Input Type to Textbox.
