Intake form Signature section not showing

    Hello, on my form I have updated it to include a new portion for the client to sign regarding informed consent regarding tx during COVID19. When I download the form as a PDF the client's signature and name is not showing up. Also, the additional informed consent part is not showing on the correct page.  The name of the file is PP Pediatric intake Form

    We do apologize for the inconvenience that you are experiencing.

    After checking your account, I can see that you have many active forms and I am unaware of which form is causing the issues. Can you kindly provide us with the URL of the form in question?

    However, based on the description of your issue, it sounds like perhaps you have not mapped the sections correctly in your PDF. Here is a quick screencast to demonstrate how you can map you from fields over to your PDF.

    Please make sure that if you are adding new fields to your form, that you map them to a section of your PDF so that the fields will appear on the original, as shown in the screencast above.
