Account suspended and forms disabled. I cannot login and my forms are unavailable.

    I was working on a form, it logged me out all of a sudden, and now I can not log back in.  My forms are also not available or showing up on my website.  Please assist.

    Hello Taylor! Your account has been automatically suspended by our Phishing Detector as you're collecting SSN numbers from your users in your form titled "Request for Taxpayer Identification Number and Certification" (screenshot attached for your reference).

    Our terms don't allow our users to obtain sensitive information from general public: Phishing. You agree that JotForm Inc. may terminate your JotForm Service immediately if a form’s purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    If you're not collecting this information from general public but from your own clients, then you need to install two required widgets on your form:
    1. Add the Terms & Conditions widget (or the Short Scrollable Terms widget) for your users to read and agree.
    2. Add the Signature widget so your users can sign and agree to provide such sensitive information.

    I noticed that you already have a Signature widget on your form. If you're okay with installing the above mentioned widgets on your forms, then we can re-enable your account after deleting the offending fields (specified above).

    You can afterwards re-install the SSN fields after implementing the Terms and Signature widgets.

    Let us know how you want to proceed with this.

    Yes I will add the widgets to my form.  How do I remove the social security number field for now so I can get the other added.  I can not login.

    Your account got suspended again because of the copyright logo on your forms.

    Please remove the copyright logo. I activated your account again. Please add the widgets above. Please update the logo on your form or show us any proof that you can use the image.

    I will be working on the widgets soon for the Social Security Number. I have attached a letter from our company for the logo release. Please let me know once received and that we can use the logos on Jotform.

    I can see your account twjuneau is currently active, you may go ahead and edit your forms as suggested here. 

    Regarding the logos, I would recommend you to update your account email address with a corporate email, this way we can verify your activity and keep your account from being suspended.
