My account has been suspended

    I am getting a message that my account has been suspended. I forgot my username/password so I went in to retrieve my password. Said that a link would be sent but I haven't received it yet and it's not in my spam.  Can someone help to open up my account?

    Your account was suspended because one of your form asks for a passport number. As per our terms of use

    Collecting Sensitive Information. You may not use the Jotform Service to collect certain types of sensitive information, including but not limited to, credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of Jotform, Inc. including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information.

    We have unsuspended your account and deleted the passport field. You should now be able to use your account now.

    Please note that for forms asking this type of information, you need to comply with two things.

    1. Add Terms of use widget (for your users to read and agree). This will surely make your forms not to be considered as guilty of phishing activities.

    2. Add a Signature widget where your users sign and agree to provide such sensitive information.
