Linked to google sheets

I have my form linked to google sheets and it is not updating properly. Please help.

My form loads data directly to a google sheet. It is set to update at any change. The form updates correctly for a few days, then all of the sudden it stops working.

Please note that if any change is made to the integrated sheet (i.e. editing column headers, adding/deleting rows and columns, adding formula, adding worksheets, etc), the integration will get broken.

Hence please reintegrate Google sheets into the form. This will pull in all data correctly in Google sheets.
