    Hi,

    We were working on our forms yesterday & JotForm blocked our IP address at work. Please unblock our IP address & unlock our account. All activity was not phishing or spammy.

    Hello Lance! Your form "Engine Technician Applicant" is collecting Driving License numbersPlease note that collecting IDs fall under sensitive information category and we don't allow it unless you add two mandatory widgets in your form which are mentioned below:

    1. Add the Terms & Conditions widget (or the Short Scrollable Terms widget) for your users to read and agree.
    2. Add the Signature widget so your users can sign and agree to provide such sensitive information.

    If you're okay with installing the above mentioned widgets on your forms, then we can re-enable your account after deleting the offending field.


    You can re-install this field after implementing the Terms and Signature widgets.

    Let us know if you want us to proceed with this.

    Hi Patrick,
    Any help you could provide to get our account unblocked would be greatly appreciated. We will never ask for drivers licensed numbers again 🙂

    Hi Lance! Upon manually reviewing your account and the form, we found that the error was indeed with our system which wrongly interpreted your form and suspended your account. We made the necessary adjustments and re-enabled your account with no changes. You don't need to remove any field from your form and are good to go.
