Account suspended

Hi there, my account was suspended for suspecting phishing activity. I believe this is because I created a form recently for collecting users' passwords. There is not any malintent behind this, rather standard practice in our industry. Please let me know if you have any questions or if there's other helpful information I can provide.

We apologize for inconvenience. I checked on your account/username kjaggers  and I noted it was suspended due to the log in form on the account. Log in forms are not allowed as it violates the Terms of Use.

We can reactivate the account only after you comply to the terms and agree to delete username/password fields in the form.

Hello, sorry about that, I didn't realize this was part of the terms. I have reviewed them and agree to delete the form once my account is reactivated.
