Google Sheets Integration: My form's Name fields no longer populate the data

My form's Last Name & First Name fields no longer populate the data to my integrated Google sheets. How can I correct this?

Thank you for reaching out.

Your form's labels are bound to your Google Sheets' column headers. If you modify one on either side, the integration will break.

If that's the case on your form, simply redo the integration so that it can rebuild the link between the form labels and the sheet headers.
