The PDF did not connect.. several non fill-able areas are missing

Hello,
To my understanding, the PDF didn't connect properly and there are missing non-fillable areas. Could you please share the PDF link so we can check it?
Also, please share the form link so we can clone it and test it on our end. Thank you!
