Hi there,
Looks like your account status was set to Dormant. Usually, it happens when an account is not used for a long time. I've reactivated your account so you should be able to see your form now.

Thanks vick!! Sorry for the duplicate questions. Yeah I used jotform for a very long time. Love it! You may now closed all my ticket.
