Symptom Calculation Form

I'd like to create a form that allows users to select which sinus symptoms they're having (out of a list of 46) and then utilize conditional logic to assess which sinus condition (out of 5) they are potentially experiencing.

Is it possible to request that this form be created if I provide the data?

If you want to build a form that can display sinus conditions based on the user's answers, you certainly can do this with the forms conditional logic. This will require that you insert 5 paragraph fields into your form where you display the information for each sinus condition and then show only the one that correlates to the user's selections.

This can be down with the Hide/Show Fields condition in your forms. Please review the flowing user guide for assistance with this feature - How-to-Show-or-Hide-Fields-Base-on-User-s-Answer
