Problem with form conditions

Hi,

I am just wondering if you could please help me with a problem i an having with a form i have created.

I have put in the conditions and when testing through all works fine, however when i publish the form and open a new link the conditions are not working. So for example, when you complete an initial review it should hide the 6 month and extended review as set out in the conditions but it doesn't seem to be working.

Are you aware of any issues or could you please take a look at my form and advise why this is happening.
