Unable to access the forms

Could you please let us know the form in question? I see there are no forms in your account acorndtc.

payment came out of my paypal account yesterday. *
I have found all the forms. It took me 2 hours. This is the link

this the email account that this is connected to,* as i am keeping the
jotform site open all the time in case it disappears again.
