Autoresponder email information missing

Typically when I use the autoresponder email feature it automatically has all the form questions and answers in the email. For some reason I have a form that only has the form title in the autoresponder email. How do I correct this?

In order to fix the issue, please add a new email notification under the Settings > Emails > Add an email.
