Hello,
My registration form for membership with my symphony is disabled on our website. Do you know why this would happen and how to fix it? We are hoping to have this resolved as soon as possible.

Our form reviewer team suspended your account because your form is collecting a password. For more information, please check our terms of use: https://www.jotform.com/terms/
We need to remove the bad form fields on your form before we can activate it. If that is okay with you, please let us know. Thank you!
