Why my account has been suspended?

     https://form.jotform.com/202988589502166 - That form was used to obtain the employee information for our company. Is this the problem? - We used and translate one of the template provided. If so, do you guys have any other option to facilitate this for us?- Thank you.

    Your account is suspended as you were collecting usernames and passwords in your form. Collecting Passwords from the users which are against our Terms of Use.

    Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.
