Why can't I create a form?

Hey,

I'm trying to create a form using the form fields in the URL below, however I can't get past 'Drivers license #" before Jotforms shows an error page.

Is there a reason for this? I am creating this form for a client who is a finance broker - so these questions will be necessary.

Driver license is considered a sensitive data and it violates our terms of use. That is the reason why the field is preventing the creation of the form.

If your purpose is not to collect these data from the general public, but from your customers, then you need to comply with two things:

1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

3. Otherwise, you must enable form encryption on your form if you need to gather critical personal information
