Can I collect photos in a form and insert them into a PDF I uploaded?

    I uploaded a PDF document that I want to turn into a form. One of the things I want the form to collect is a photo of a person, which would then end up showing in the final PDF document. Is this possible to do in Jot Form? I got it to work when I built a form from scratch, but couldn't get it to work with a PDF I imported.

    Hello,

    To clarify, you would like to let your users upload their photo on the PDF Fillable form and show it on the PDF submissions, is that correct?

    Yes, this is possible by adding the File Upload field (that you have used on your form) on the PDF Template. You can add the fields that you have used on the PDF Fillable form from the Form Fields tab in the Add Element panel :

    Hi,
    I tried to test this out, and I am not seeing the options in your screenshot when I am trying to build the form and map it to the PDF we uploaded. See below screenshot. I was able to create a new PDF using the PDF editor and pull in the photo I had already uploaded, but really we are interested in mapping the photo to the PDF so that it automatically flows through when the person filling out the form enters it. Am I missing something or maybe not understanding the instructions? Thanks so much for your patience. I’m new to JotForm and trying to figure out if this will work for our process.

    Unfortunately, it is not possible to link the image element in the original PDF.

    My colleague was referring to the PDF document that was generated as an output. When a form is submitted the original PDF with the collated data and the PDF document created by JotForm is generated at the end.

    I believe you were referring to the Original PDF. Unfortunately, it is not possible to link the image to the PDF.
