HIPAA form is not loading

    I have checked your form https://www.jotform.com/200925378859065 and it seems to be loading.

    To clarify, are you not able to submit the form?

    Can you please share the error message you receive?

    I have tested our HIPAA account and I can't seem to replicate the issue at my end.
