sports event registration/event form

Hi, I want to have events where I can have one player at a time participate and have the registrants pick a time slot during the event day and then will be blocked by others after its chosen as to not have doubles in that slot. I would then like to ck in on the event on the days prior to the event to asses the quantity and age groups that will be in attendance.

is there a template that meets this criteria or do I have to create one?

I see that you have already created an appointment form. This should be enough to register data, players, age, and every information from your form. Then you can access these submissions using JotForm Tables. In the screenshot below, you can see how.

At JotForm Tables, you will find a lot of features to analyze your submissions. For example, you can filter by date and see the quantity and age groups for that day.
