    Are you referring to the form elements or the page breaks?

    Hey Vincent,

    Thanks so much for the reply!

    The issue I'm running into is creating a condition for what I thought was page 14 (Add Network Drives & Folders), however it doesn't appear to be an option on the dropdown. I have a feeling I've set things up incorrectly, and hoping you can set me straight :)

    Hi Bryan,

    I cloned your form and if I select the "Driver/Folder" option, it will skip the page to 14. Here's a screencast:

    I need each choice under "Request type" to go to a different page.

    However, this is greyed out when I access the conditions.

    Hi Bryan,

    I checked the form again and the "8. Request Type" is a header field. Please check my screencast below, I updated the Single Option and added a form label to appear in the Skip Page condition:

    That's the reason why you cannot select it. Please try following the screencast and let us know if the issue still persists.
