Scale rating: a need for a numeric increment step

    Can anyone think of how to add a scale like this one below?

    specifically the text next to the scale 'very well' and 'very poorly'

    This is a good option, but we have to have a 0-100 scale in 5 increments..

    Any other ideas?

    The other option is the star rating, but the layout would be like this:

    You may change the background design by injecting a custom CSS, but there are default styles that you can choose from. Although, not similar to what you have in you screenshot.

    Adding to my colleague, you can also apply the following work around to achieve an output that is very near to the desired one, this will be by using an Input Table:

    1- Add an Input Table to your form then click on its gear icon to edit it.

    2- Go to FIELDS then select Radio Button for Input Type.

    3- Clear the Rows field and put your text for the scale.

    4- Clear the Columns field and type down your scale numbers, each number in a new line.

    We will communicate to our developers the need to add a numeric increment option to the Rating Scale element so it would be available in the near future.

    Adding to the possible options, I strongly suggest using Fancy Range Slider widget as I believe this scale the most relevant to your needs, check the this screencast to know how to add it & configure its settings.
