Having fillable PDF Form on website

I have a fillable PDF form that we would like to use on our website. We were referred to your company. We would like the PDF to look exactly the same on the website. Is this something that can be done or would each form be tailored to your appearance? For instance, I uploaded the PDF but when it "converts" the font, style and look of our form is all gone. Is this what happens no matter what (outside of adding a logo at the top)?

    Thank you for contacting us.

Please note PDF forms can not be embedded. However, you can import your PDF Form to JotForm and embed the online version of your PDF Form. The online version of your form can be connected to your original PDF form, which would allow you to download the submitted data in the same format as your original PDF.

Also, please note that our online forms are not designed to look like printed forms. After the conversion, your online form might not look like your original uploaded PDF.
