How are people requesting access to my submissions?

How are people able to request access to my submissions, I am not announcing the link anywhere im not sure where they are getting the link from?

As we check your JotForm, we notice that one of the autoresponders sent to the form submitters is having a QR code added to access their form submission.

Since the account is HIPAA enabled, the protected information cannot be accessed directly and thus sending a request to you to access the submission. You may remove the QR code from the email to avoid this and add the form fields directly in the email so that the non protected fields can only be displayed. Please see the guide below:
