How to have multiple appointments for one slot?

on an appointment form, how can i offer multiple slots for the same times? for instance, i want to offer 4 slots for each 15 minute appointment.

To achieve this, you can enable Group appointment type from the Advanced settings:

In the Maximum Attendee Per Time Slot option you can place the number of appointments you can have in each slot.
