I need personalization forms for trophies, signs, etc

I offer hundreds of styles of awards that require custom engraving, so customer selects a trophy/award/sign, etc, etc, then selects size, then selects color, then sends us engraving, maybe needing a selection of fonts, sizes, colors (in signs)

Yes it is possible to build such form in JotForm. You can add "Image Radio Buttons" widget to list all the trophy/award/sign and then display the question for user's customization.
