I did check your form https://www.jotform.com/form/203428030647046, and the original PDF is not showing. Can you please reupload the PDF to see if that'll resolve the issue. I have cleared your form's cache as a precaution.
If the issue still persists, please create a form and import the PDF file again to see if that'll make any difference.
