I have cloned and tested your form,
The textbox field is filled out on the PDF.
I have checked your form and you just need to connect the fields to the PDF.

This is not what I mean, I mean when the respondent fills out the form, text appears inside the field for him to write in and not leave it. Also, the problem of Arabic letters is still not resolved, as in the image below
