    Hello Philippe! There are two issues with your forms. These are mentioned below.

    1. First issue is that you're collecting credit card information on your forms (screenshot attached below).

    We do not allow collection of credit card information on our forms. Payments can only be collected via the payment integrations: mastering-payment-form-integrations-with-jotform

    In case you only want payment authorization, then you can use these payment gateways: how-to-enable-payment-authorization

    We'll need to delete these payment fields from your forms to enable it.

    2. Second issue is that you're also using Copyrighted logo on your form (screenshot attached below).

    Your form could be violating the copyright law by using the image logo. Can you give us more information that you are associated with this company/store?

    We understand this might not be the case with your form, yet we had to act according to the general rule to be in compliance with our Terms: Free accounts with non-corporate emails showing suspicious forms must be suspended immediately. 

    In order to reactivate your account, kindly provide us the following information:

    1. The URL where your forms are used.

    2. Your website or proof of affiliation to a legitimate institution.

    3. Share with us a document that will prove that you're authorized to use the said logos.

    We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

    Send these proofs to reviewers@jotform.com with Thread #2786272 as your subject. We will let you know about the status of your account after a thorough review of your proofs.

    Or, we can just remove the logos and lift the suspension of your account.
