Cannot upload Fillable PDF

Hi tried uploading a smart pdf form and its just not uploading it, its only 128kb ?

any ideas on how to get it loaded ?

wouldnt let me send without adding a picture ??

Please make sure that your PDF has fillable fields so that it would be uploaded to the Smart PDF forms.
