Can we do a form by jotform?

The form's sections:

Title of the trip - dates - number of days - serial number

1.Personal traveler details (details below), also includes the ability to add flight details, or upload an e-ticket.

2. Other travelers' info, including option to add a separate flight or share a flight with a passenger. This includes all details we had for the main traveler. Once they put in their flight info - open a checkbox offering them a payable transfer from the airport transfer

3. Select number of rooms. Each room should have the choice of single/double/twin or triple, with icons showing the room layout. Each room should have:

a. list of participants, so they "attach" room to the people sharing it.

b. list of optional upgrades per room, including prices (that need to calculate with a total sum), We provide this info. Every option for an upgrade should include a photo of the room, price, and a sort text (or title is enough, if it's easier)

4. Upsell option - should include an unlimited amount of boxes (2-3 per line, on a desktop layout). Each box should have: title, image, price, description, add to cart. (once we have the template for it, we'll put the info in)

5. Terms and conditions - 3 checkboxes,

Read and understood T&M, I approve my info, and specifically approve the cancellation policy.

6. On the bottom - signature and Review answers before submission.
