Unable to submit my form

Hello:

I did some editing to the form and it appears it is not published?

The link appears correct but patients are unable to submit it.

Hello Rob,

I've checked your form and found that the Submit button is missing, you will need to add it as in the below screen cast, the Next button is only used to navigate to the next page of your form.
