Hi,
The properties are all there in the email notification but do not receive them in the email. I add them and save it and they appear then they disappear. It's like the last 6 fields.

I cloned your form and tested it. As a result of the test, I couldn't replicate the issue. The fields came properly in the Notification Email.
Yet, I cleared your form caches. Please check again and if the problem persists, please allow us to send test submission to your form.

We apologize for inconvenience. I checked the form Email settings and it appears that the existing Notification email template was broken.
I suggest you try first reset the Notification Email template by deleting and adding a new one.
Let us know if issue still persist even after doing these.
