When I try to preview the form I get a "404 Not Found" error. I'm using chrome and Firefox and I have same issue.

    Hi @udmdentalcp,

Thank you for reaching out to us. I just checked your form in preview mode and I was able to preview the form successfully on my end in both Chrome and FireFix browser.

I also checked your previous messages and It seems you are seeing 404 error messages before. May I ask if you still see those error messages after you changed your internet connection(Switches to mobile phone tethering/hotspot), clear your browser cache, disable your computer security software temporary to see if the situation would improve? This might be caused by local environment, so if the issues persists, please try to contact your IT department to have a look at this issues as well.

If you also have any other questions or require further assistance, please reach out to JotForm support again.

    Best,

    Yuta

    Him,
It's very random. I try to access it after making some edits it shows the error and sometimes it goes through.

    Hello @udmdentalcp,

Thank you for sharing the additional details.

Since you are mentioning that this issue is intermittent, it is most likely being caused by an internet connection error that causes the page to not be found. I have tested your form on my end from several browsers and two different devices and I was not able to replicate the issue.

Next time this happens, I would suggest that you immediately take a screen capture and then using a different device under a different internet connection, try to open the same form in preview mode to see if the same issue occurs. Please get back to us with your test results.

Also, it seems that the live form is functioning correctly, so there shoudl be no issues with submitting your form.
