Create online form out of PDF forms.

Forgive my ignorance, my employer has to change from pdf to html forms. I am not in IT or know coding. Does your product create HTML forms? And can they be integrated into an electronic health record system? I appreciate seeing that they are HIPAA compliant.

Kindly note that to properly upload your original PDF form to JotForm to populate it using normal form fields, you may import it by clicking the "Create Form" button on your "My Forms" page, then follow the steps below:

Afterwards, you will be redirected to upload your PDF form, then you may easily link its fields to a form you would create using Form Builder.
