    When trying to sign up for a covid vaccine with a pharmacy that utilizes Jotform, I somehow opened an account with Jotform rather than with the pharmacy. Please see that the account is closed immediately and notify me that it has beeen dropped. If there is a name and phone number of an individual that can help me with this issue, please send that information to me.
