How to make a clone unique to the job site?

Hello. I would like to create an attendance form for my jobsite. Yes, simple enough lol. This is all great for my site but now other ppl within my company would like the same convience at their jobsites. is it possible to create a clone of said form in such that other safety managers at other jobsites can get the reports the form would generate specific the their site directly to them inlieu of me forwarding daily reports?
