Sending Notifications to multiple email addresses

Hello,

I currently have the free version of jotform. I'm looking to upgrade but i want to find out which level i need.

Here's the scenario. I only need 1 form and it's very basic. However, i need it to send the notification to different email addresses based what choice is chosen on the form. For example:

Question 1 is what trucker?

Options are:

Dave

Kurt

Sam

If the trucker is Dave, i need the notification sent to his email. If it's Sam i need the notification sent to his email.

Which level of service would be best? We have about 50 truckers.

Any of our paid plans will allow you to set up multiple email templates, add multiple recipients, and set up multiple email conditions—for example, the Bronze plan.
